Privacy Attestation
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) prohibits Protected Health Information (PHI) from being shared without permission from the individual except in certain situations. As a Community Assistor, you are attesting you received permission from an individual to access that individual's PHI. After receiving the authorization from the individual, DPHHS shall not be responsible for any disclosure you make of PHI you have access to thereafter. This authorization automatically lapses after each session with a Community Assister. Community assisters are prohibited from signing any application or document on behalf of a citizen and must ensure the citizen provides their own signature at all times.
